Passport Applications
You Can Renew Your Passport By Mail (No Appointment with Us is Necessary) If Your Most Recent Passport:
- Can be submitted with your application*,
- Is not damaged (other than normal wear and tear),
- Has never been reported lost or stolen,
- Was issued within the last 15 years,
- Was issued when you were age 16 or older, and
- Was issued in your current name (or you can provide a document like a marriage certificate or divorce decree to show you have changed your name)
Tips to Complete Your Form
- Complete all parts of the form. Sign and date your form before you mail it.
- Print your form on single-sided paper. We do not accept double-sided forms.
- Check your date of birth and previous passport issuance date if you did not get the form you expected to receive on the form filler.
- You can renew a , a , or both documents.
- Use Form DS-82 to apply for your first passport book if you only have a passport card, or to apply for your first passport card if you only have a passport book.
- If you travel many times each year, get a larger passport book with more visa pages, at no extra cost. Check the large book’ box at the top of the DS-82.
- If renewing from Canada, do not include your Canadian Social Insurance Number on the form in place of your U.S. Social Security number.
You Will Need to Make an Appointment and Apply for a New Passport If:
- I am applying for my first U.S. passport book or card.*
- I am applying with my child who is under age 16.
- My most recent U.S. passport was issued when I was under age 16.
- My most recent U.S. passport was lost, stolen, or damaged.
- My most recent U.S. passport was issued more than 15 years ago.
ÌìÃÀ´«Ã½ Passport Services is a Passport Acceptance Facility for the U.S. Department of State. Although the facility is located on the campus of the ÌìÃÀ´«Ã½, any U.S. citizen may obtain their passport from this location, regardless of ÌìÃÀ´«Ã½ affiliation. Free, convenient parking is available.
Passport Photos
Passport photos are not available on site at this time. Please follow the instructions below provided by .
Photo Tips
- Submit one color photo
- Submit a recent photo taken in last 6 months
- Use a clear image of your face
- Do not change your photo using computer software, phone apps, filters, or artificial intelligence
- Have someone else take your photo. No selfies.
- Take off your eyeglasses for your photo
- Use a white or off-white background without shadows, texture, or lines
Appointments
ALERT: PLEASE COMPLETE AND PRINT OUT YOUR APPLICATION FORM BEFORE YOU MAKE AN APPOINTMENT.
PLEASE NOTE:  WE CANNOT PROVIDE PHOTOS AT THIS TIME – PLEASE MAKE SURE YOU BRING A PASSPORT PHOTO WITH YOU.
PLEASE DO NOT SCHEDULE AN APPOINTMENT UNTIL YOU HAVE READ AND PREPARED THE FOLLOWING:
- A completed Application, form DS-11, available atÂ
- Proof of U.S. citizenship – an expired passport (no photocopy necessary), original or certified copy of U.S. Birth Certificate, Certificate of Birth Abroad, or Certificate of Naturalization and a photo copy of the document.
- Photo Identification – Driver’s License, Government or State-issued ID card and a photo copy of the front and back of the card.
- Application Fee ($130.00) – Payable to U.S. Department of State by check or money order only.
- Execution Fee ($35.00) – Payable to ÌìÃÀ´«Ã½.
- Passports for Minors (Under 16 years of age) – Both parents, along with the child, must be present with photo ID.

If you already have a passport, you may be eligible to renew it by mail.
